Mandibular Third Molar Impactions

This course introduces learners to the diagnosis, classification, and surgical management of impacted mandibular third molars. Learners will interpret panoramic radiographs to classify impactions using the Winter and Pell & Gregory systems, evaluate indications and risk factors before extraction, including proximity to the inferior alveolar canal and review the surgical steps and for removal. The course is curated for classroom-integrated, self-paced learning and is not a substitute for supervised clinical training.

About Course

This course provides a comprehensive introduction to mandibular third molar impactions, covering their etiology, incidence, and commonly used classification systems. Learners will explore Pell and Gregory and Winter’s classifications to assess impaction patterns and surgical difficulty. Classify mandibular third molar impactions on a panoramic radiograph using the Winter (angulation) and Pell & Gregory (depth and ramus relationship) systems.
Evaluate radiographic and clinical indicators of surgical difficulty, including proximity of the roots to the inferior alveolar canal (Rood’s criteria).
Explain the accepted indications and contraindications for surgical removal of impacted mandibular third molars.
Outline the sequence of a standard surgical extraction, including flap design, bone removal, tooth sectioning, and wound closure.

Prerequisites

To be successful in this course, learner should be able to:

Basic oral anatomy, particularly mandibular anatomy and the inferior alveolar and lingual nerves.
Basic dental radiographic interpretation, especially panoramic radiographs (OPG).
Principles of exodontia, local anaesthesia, asepsis, wound healing, and basic postoperative care.
Basic knowledge of oral surgery instruments and surgical principles.

Who Can Take This Course?

This course is for final year BDS students and junior dental trainees who are developing competence in the assessment and management of impacted mandibular third molars. It is particularly relevant to learners preparing for clinical oral surgery and OMFS rotations, examinations, and supervised clinical practice.
